  A 47 year old patient,House wife by Occupation,Resident of Nalgonda,Came to OPD with cheif complaints of Generalized itching since 1 month. Difficulty in breathing since 7 days. Back pain since 7 days. HOPI: Patient was apparently asymptomatic 6 years back then she developed giddiness for which  she went to Hospital and diagnosed as diabetes and she was on regular medication. In March 2022 there is a history of edema in both lower limbs up to thighs ? for which she went to hospital in Nalgonda and was discharged after resolving edema? also in her tests she was  found to be having Hypothyroidism and Hypertension and she was started on regular medication. In April 2022 she was having Abdominal Distension,She diagnosed as Ascitics for which ascitic tap was done .At that time on further examination she was found to be have cervival lymphadenopathy. In June,FNAC was done? and found to be have extra pulmonary tuberculosis. She is on ATT from 21 july. H/o generalized itching...

70 year old male farmer by occipation, Resident of Nalgonda came to hospital with chief complaints of inability to lift left upper and lower limb since morning. HOPI: Patient was apparently asymptomatic 20 days back then he developed generalized weakness. On 24 August 2022: After his dinner at 8 PM,he went to bed 9 PM. On 25 August 2022 Morning when he was about to wake up from the bed,he could not lift his left Upper and Lower limb which was Sudden in onset and there is loss of speech since morning then he was immediately Hospitalised. There is no H/o fever,head injury,Vomiting Loss of consciousness, palpitation,chest pain. H/o bilateral pedal edema 7 month back for which he went local hospital then he diagnosed with Right mild hydroureteroephrosis,Gall bladder slugde with calculi,Fatty Liver. PAST HISTORY: No similar complaints in past. He is known case of diabetes and Hypertension since one year and was on  regular medication.  No H/o Tuberculosis, Epilepsy,Asthma. PERSONAL...

 AMC BED.NO-5 52 YEAR OLD MALE  Date of admission: 20/08/2022  A 52 yr old male patient came to OPD with  Chief complaints: -fever since  2 days  -slurred speech for half an hour 2 days back then he regained speech -bilateral pedal edema since 2 days -abdominal distension since 2 days -decreased appetite since 2 days  -decreased urine output since 1 day Pt was apparently asymptomatic 30 days back then he developed pedal edema B/L pitting type for which he used herbal medicine and edema got resolved.  -10 days back he again developed B/L pedal edema pitting type associated with abdominal distension and decreased urine output for which he was treated constantly.  -Since 2 days he had c/o fever , low grade , intermittent a/w pedal edema pitting type B/L till knees , abdominal distension and stool urine output (reddish)  *H/o seizures  - one episode in childhood, - one episode 30 yrs back  - one episode 10 yrs back - 10 months back...
